Patients wishing to register with us on a permanent basis must be a resident in our catchment area which covers Pinner, North Harrow and Rayners Lane specifically
If you have a new baby you are trying to register, you can attend at any time and we will register them as a priority. As soon as you have completed the registration forms and given them to us, you are a registered patient and can access care and treatment from us.
Those registering will then be attached to the Ridgeway GP Surgery where most appointments will take place. Patients can pre-book appointments at our polyclinic but should expect to have the majority of their primary medical care at the Ridgeway Surgery.
Like all NHS practices, we are restricted to taking patients from a limited catchment area - Catchment Area Living in our catchment area is important to enable us to provide the best service in the event you may need a home visit - see map below.
How to Register
Once you have confirmed on the above link that you are in our catchment area, you can print and complete the two forms below for each person registering.
You should also bring proof of your identity (such as a passport) and proof of your address (such as a utility bill, council tax statement or tenancy agreement)

You can return these forms to us on Tuesdays between 10am-4pm at the Ridgeway Surgery or Alexandra Avenue clinic.
We will unfortunately not be able to accept Registrations on the following Tuesdays in 2020:
