With effect from 1st January 2020, The Ridgeway Surgery requires pre-payment for ALL non NHS work .
Please note that if you want copies of medical records you can access these online subject to GP approval by completing our Request for a non NHS report to access these. If for whatever reason this form of access is not suitable for you, please complete the form indicating what do you require and for what purpose. We will then assess your request.
Under the General Data Protection Regulation (GDPR) – 2018 all patients can submit a Subject Access Request (SAR) to access their medical record, and we as your medical provider are obliged to release them for no fee, however, the act states that that we have 28 working days to supply your records .
All non NHS reports/ letters normally take between 7 -10 working days to complete. This may take longer if the doctor needs to discuss your request with you. If you require the report/letter as urgent, please note this carries an additional payment. Please ask the reception to check to see if this is viable (due to available clinicians).

* £15:00 – private sick note
* £30:00 – private letter, claim form, health report. If the request is very complex, it may incur an additional fee – you will be advised at the time
* £50.00 – URGENT request within 48 hours -please refer to the receptionist to check the viability of the request
*£110.00 – medical report – Transport of London – TfL medical questionnaire /PCO / DVLA/ or similar – this will need a 20 minute appointment and the opticians section of the request to be complete prior to seeing the doctor.
