We are happy to register new NHS patients within our practice boundary area.
How to Register
To register with our practice, please follow the link below to complete the online registration form.
After you have completed and submitted the form you must the complete the relevant form below, and email the reception team within 7 days with two forms of identification and the completed document.
If you cannot provide the documents requested, please provide whatever ID documentation you are able to so we can assess if can accept it.
We also request that you click on the below link in order to join the surgery patient group. This will ensure they can keep you updated about patient events, changes at the practice which may affect patients and act as your voice.

Acceptable identification includes:
- Passport
- Birth Certificate
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration/mail forwarding letter.
If you are in our catchment area and are only going to be residing in the area for 3 months or less then please complete the temporary registration form and email it to haroccg.ridgewayrecepinfo@nhs.net GMS3
